A: ElderCare Consultants is a professional care management company that started in 1998 by a registered nurse and licensed social worker.  After 20 years of experience working in a hospital setting, they addressed the need to provide ongoing care for clients beyond the inpatient setting.  They responded by forming ElderCare Consultants, providing mental health and geriatric care management services community-wide.

Our multi-disciplinary team consists of Registered Nurses, a Certified Holistic Nurse, Licensed Psychologists and Licensed Social Workers, who specialize in geriatric care and behavioral mental health.  Throughout its existence, ElderCare Consultants has successfully helped hundreds of clients and families navigate through complex medical and social service systems.

Q:  What is a Geriatric Care Manager? 
A:  A Geriatric Care Manager is a qualified professional who can help you or your family member to manage your medical, psychological and social affairs, and assist you in coordinating your financial, legal, and real estate needs.  Facing a major life transition or coping with chronic illness is often an overwhelming experience for both individuals and families.  Inter-personal or familial conflict is often a natural consequence of significant life changes.   Geriatric Care Managers understand this process, and can help you navigate a complicated medical and social service system, while advocating for your rights and wishes. 

Q:  What services can be provided by a Geriatric Care Manager? 
A:  Our team of care managers can: 

  • Perform a comprehensive assessment of a person’s strengths and needs in the areas of:  physical, cognitive, emotional, social, financial, and legal situations.

  • Provide medication management.
  • Provide client/family counseling and support.

  • Incorporate Holistic Nursing Care services along with traditional medical and social services.
