- Navigate senior housing options, accompany you on tours of your choice, and coordinate and facilitate the entire moving process.
- Provide medication management.
- Provide client/family counseling and support.
- Facilitate and coordinate financial or legal services, including estate planning, guardianship, conservatorship and power of attorney.
- Provide medical oversight and care coordination with family and doctors.
- Incorporate Holistic Nursing Care services along with traditional medical and social services.
- Screen, arrange and monitor in-home care or related services.
- Communicate with long distance family members to be the “eyes and ears” for their loved ones.
- Provide crisis intervention and around the clock accessibility
- Offer a professional travel companion through our Safe Passage Program
Mental Health Care
- Assist with psychiatric diagnosis differentiation
- Access the most appropriate and cost-effective psychiatric care
- Promote independence by utilizing the least restrictive level of care and residency
- Partner with families, doctors and other mental health professionals to coordinate care and promote wellness and independence.
- Problem solve matters involving probate and mental health court.
HELP—Housing Experts Location Program
Perform a comprehensive needs assessment to determine level of care needed.- Arrange for necessary care to keep you in your home, or help find an environment that promotes independence and wellness while providing for all your physical, social and emotional needs.
- Provide a list of potential housing options.
- Accompany you on tours of your choice.
- Coordinate and facilitate the entire moving process from start to finish, including estate planning, financial planning, organizing, packing/unpacking and adjusting to your new environment.
